For a limited time, you can earn a referral bonus for helping us bring in new team members.

⏰ This referral program is available for a limited time and expires April 30, 2026.

Earn $200 for every successful referral.

The referral bonus will be paid after the referred employee completes five (5) full pay periods while maintaining the required work hours.

This referral program applies to new applicants starting March 1, 2026.

Applicants who applied or were in the hiring process before March 1, 2026 are not eligible for the referral bonus program.

To qualify for the referral bonus:

• The new applicant must list your name on their job application when applying
• Referrals must be listed at the time the application is submitted
• Referrals added after the application is submitted will not qualify

If multiple individuals are listed as referring the same applicant, the first name listed on the application will receive the referral credit.

To qualify for the referral bonus, the referred employee must:

• Maintain an average of at least 15 hours per week during the five pay period qualifying period
• Only hours worked providing services to active clients count toward the minimum hour requirement
• Complete five (5) full pay periods of employment
• Maintain good standing with the company
• Maintain reliable attendance and accurate timecard submissions

If the referred employee resigns, is terminated, or falls below the minimum hour requirement before completing the qualifying period, the referral bonus will not be issued.

The individual who referred the candidate must remain actively employed with HealthChoice at the time the referral bonus is issued.

Both the referred employee and the referring employee must be actively employed at the time of bonus payout in order for the referral bonus to be issued.

We are currently prioritizing referrals in the following areas:

• East Metro (St. Paul, Inver Grove Heights)
• South Metro (Prior Lake, Burnsville, Eden Prairie, Shakopee)

Referrals outside these areas may not qualify depending on current staffing needs.

• Only one referral bonus may be issued per new employee
• A referral does not guarantee employment
• All applicants must successfully complete the hiring process
• HealthChoice reserves the right to determine eligibility for referral bonus payments
• HealthChoice may modify or end the referral program at any time

Tell them to apply and make sure they list your name on the application.

If they join the team and meet the program requirements, you earn $200.

There is no limit to the number of referrals you can make.
